Osteoporosis is a significant health crisis facing women’s health. It affects more than 25 million Americans, with over a million osteoporotic fractures occurring each year. Unfortunately, 10% of these patients will die within one year after sustaining an osteoporotic fracture. If the patient is fortunate enough to survive, only 50% will regain their former level of functioning. If these statistics are not eye-opening enough, it is estimated that one-third of these survivors will become nursing home residents. The estimated medical costs associated with osteoporosis are over $14 billion.

Now that I have your attention, my question is what are you doing to prevent osteoporosis? What is your dual-energy x-ray absorptometry (DEXA)? What is your NTX score? Are you performing regular weight-bearing exercise? Do you have an adequate intake of Calcium and Vitamin D? Are you taking Strontium, Vitamin D, or Vitamin K?

Although all of these strategies have benefit, it’s actually the use of estrogen that has been considered to be the agent of choice for both prevention and treatment of postmenopausal osteoporosis.

The DEXA scan is considered the “gold standard” for measuring bone mineral density. However, it is important to note that the DEXA looks only at past bone metabolism. I order the NTX test on my patient’s undergoing estrogen replacement because it looks at future bone metabolism. In other words, the NTX test helps predicts what bone mineral density will be, not simply showing what has happened. The NTX is advantageous because a “normal” DEXA scan can be misleading because bone loss accelerates rapidly in menopause.

The most popular treatment used today for the treatment of osteoporosis is biphosphonates, such as Fosamax. However, these medications are very unpopular with patients. These medications have been linked to side effects, including mandibular osteonecrosis, mid-shaft femur fractures, and esophageal cancer. There is no wonder why compliance is poor with these medications.

This is a list of other treatment options that have been shown to be advantageous in the prevention of osteoporosis. First and foremost Bio-Identical hormones. Also, Vitamin D3, Vitamin K, Strontium, and Ipraflavone have been shown to be beneficial.

The research supports that estrogen treatment can actually increase bone mineral density, whereas other treatments only hope to slow down bone loss. As a result it is important that patient’s stay on estrogen replacement in order for the protective benefits to last. Even if you have been post menopausal for many years, it is never too late to start estrogen treatment for bone protection.
